Periodic review of chronic treatment

Pharmacists’ roles in hospitals have expanded from medication distribution into medication management, to help assess patient medication compliance. Their roles include reviewing current therapy with intended outcomes, and formulating treatment plans based on patient’s clinical response to treatment, understanding of their medications and social needs, such as their ability to access medications. To help improve outcomes, it is important to extend pharmacist access outside of the hospital. Pharmacists are in a unique position to review patient progress with their medication therapy plan, and make recommendations for changing therapy, based on patient needs and understanding. This article discusses the importance of periodic review of medication and recommended action steps to provide this service.
